Instructions:

Project Proposal:

The following items are suggested elements for describing your project plan and are intended to provide information about your planned activities that will facilitate our review of your proposal. Please review and address these points as you see fit:

 

  • Description of the problem or need to be addressed
  • Name and qualifications of the project manager and staff considered essential for success of funded activities
  • Description of grant objectives and expected results or outcomes. What do you intend to accomplish? Provide milestones and expected dates by which they will be reached, if available.
  • Target audience/intended beneficiaries. Please tell us, with as much detail as possible, who will benefit from activities for which funding is requested and how they will benefit.
  • Plan for measuring success of your project.

 

For on-going services or projects:

 

  • A short summary of work already completed, and explanations for why additional funding is needed and how planned activities build on or differ from previously completed work.
  • For a new projects: a clear explanation of why this funding is needed, expected outcomes, the rationale for the approach chosen to achieve these outcomes, and, if applicable, other approaches considered.
  • If applicable, list of partner organizations involved in planned activities and the role each partner will play.
  • As noted in the General Information and Application Requirements, your application must have a significant volunteer component. Explain the role volunteers will play in proposed activities.
  • Potential challenges that could impact the proposed work and plan for addressing these challenges.
  • If applicable, a sustainability plan explaining how funded activities will be sustained after grant funds have been expended.

 

Budget:

Your proposal must include:

  1. Funds requested for this grant effort and total project cost. Will the funding requested through this application pay for the project in its entirety or contribute to the overall cost? If the latter, please provide detail on how additional funding has been or will be secured to complete the proposed activities.
  2. Detailed cost breakdown to account for planned use of requested funds including rationale for individual major cost categories (salaries, equipment, supplies, etc.).

 

Please note that requests for grant funding for salaries or travel expenses are discouraged but may be considered if directly related to a new or existing client service. Grant funds may not be used to support another charitable organization.

 

Financial Documents:

Your most recent IRS Form 990 [Link] or other proof of non-profit status for your organization.
